What do women think of guys with a nipple piercing?

My opinion: the piercing will take the attention from your 'hot body'. I personally, don't like piercings on nipple(s). Just my opinion. If I saw a guy with a nipple piercing I would think that he is definitely not ready to settle down and just likes to have fun and play games. If that's what you are looking for, leave it.
